At first go to this url:www.google.com/adsense‎

Then you have to click in get started button.Then you have to give your website url and then give language .Then you have to click submit.Then you will see a contact form.Give your all information correctly.Dont give fake information.Give your street address correctly  because when your account get approved ,google  will sent a code in your account.So be care full.Use a verified email account .Then click all box .Then submit.It will take some times to approved.If  you are a blogger user then you have at first you have to go in your earnings tab.Then all things are same.

Check your email address for your account confirmation code.Put this code in html section.
